Directions to our office

From Route 309 exit Hamilton Blvd South. Stay to your left to continue onto Hamilton Blvd; not the Bypass. You’ll pass the Hamilton Crossing Shopping Plaza on your right. When you’ve crossed over Krocks Rd., our building will be on the left next to the John Yurconic Insurance Agency. At the next traffic light make a left on to Shepherd Lane at the Shepherd Hills Center and then immediate left into our parking lot. You’ll see the numerals 5920 on our professional building. Please enter the building through entrance “A”. We’re straight down the hall in Suite 103.

From Route 100, take Hamilton Blvd North. You’ll pass Mill Creek Rd. at WalMart, then you’ll pass the intersection at Commercial Center Drive. The next traffic light will be at Shepherd Lane at the Shepherd Hills Center. Make a right onto Shepherd Lane and then an immediate left into parking lot. You’ll see the numerals 5920 on our professional building. Please enter the building through entrance “A”. We’re straight down the hall in Suite 103.

There is private parking around the 5920 building. Please enter through Entrance “A”. To maintain the privacy of all clients, there is no need to “check-in”. We can see you’ve arrived and will be with you shortly.

New Patient Forms

Upon contacting our office for a New Patient appointment, we will ask for your email address. You’ll receive an email from Therapy Notes inviting you to complete an account on our Patient Portal whereby all forms are sent to you and returned to us. You’ll be able to very simply open and complete each form then submit each form within your Therapy Notes Patient Portal. Therapy Notes is an Electronic Health Record which is a HIPAA complaint platform, encrypted at both ends for security and developed specifically for Psychologists.
